This gives us all the information we need to get you started. You can download and fax the form or simply send an e-mail with the information requested on the form. Once we receive this information we’ll get everything set up and send you a confirmation email for the visit.
The state of Colorado requires us to obtain written permission in order for you dog to co-mingle (play) with other dogs. You can download the form and give it to the driver when he comes to pick-up, or we can take care of it when you bring your dog(s) out for their first visit. If it’s easier to fax the form, that’s fine too.
