Make a Reservation

Use our convenient online form or call 303-367-1213 to make a reservation. If you make a reservation online know that you will not receive an immediate response. We’ll get back to you by the next business day to confirm your reservation. If your dog hasn’t been to Shylo before, continue reading below for a few additional items to complete.

Before Your First Visit

Before your first visit, we need a few things from you:

First Visit Form – This gives us all the information we need to get you started. You can download and fax the form or simply send an e-mail with the information requested on the form. Once we receive this information we’ll get everything set up and send you a confirmation email for the visit.

Permission to Play Form – The state of Colorado requires us to obtain written permission in order for you dog to co-mingle (play) with other dogs. You can download the form and give it to the driver when he comes to pick-up, or we can take care of it when you bring your dog(s) out for their first visit. If it’s easier to fax the form, that’s fine too.

Vaccination records – Please provide us a copy of your pet’s vaccination records showing that they are up to date on all vaccines, including kennel cough (Bordetella). We accept your veterinarian’s recommendation on when these vaccines should be given. We need to have these prior to your first visit, so please email or fax those to us. If you authorize your veterinarian, they’ll be happy to fax that directly to us as well.
